The following year, Sirona won a contract to provide adult community healthcare services in South Gloucestershire oresulting in 450 staff members transitioning from North Bristol NHS Trust. In October 2015, in collaboration with Bristol Community Health and Avon and Wiltshire Mental Health Partnership NHS Trust, Sirona secured a contract to deliver children’s community services in Bristol and South Gloucestershire.

The demand for medication adherence support is increasing, as many individuals require assistance in taking their prescribed medicines correctly. This issue has prompted BNSSG ICB, in collaboration with The Health Foundation, to secure funding for an innovative pilot initiative – a Personalised Medication Support Service (PMSS).
The purpose of this role is to facilitate the successful implementation of this pilot project within Sirona Care and Health. The selected candidate will play a pivotal role in supporting individuals within the Sirona caseload by helping them manage their medications more independently through the use of assistive technology.
The post holder will work in close coordination with the Sirona Medicines Optimisation Team to explore and test strategies aimed at improving medication adherence. By leveraging technology, the goal is to assess whether service users can maintain their medication routines with minimal intervention while still receiving appropriate support.
A key component of this position involves home visits to prospective trial participants. These visits are designed to:
- Provide tailored guidance on the range of available digital and technological solutions that can aid medication management.
- Assist in the installation and setup of these technologies, where appropriate, ensuring they align with individual needs.
- Conduct follow-up visits and assessments throughout the trial period to measure the success and effectiveness of the selected interventions.

Key Responsibilities
This role involves multiple responsibilities centered around assisting individuals with medication self-management using technology-driven solutions. The main duties include:
1. Providing Personalized Support & Technological Assistance
- Educate service users on the benefits of technology in medication adherence.
- Deliver tailored advice based on each individual’s needs and medical circumstances.
- Help service users understand how to use assistive technology for self-management.
2. Collaborative Project Delivery & Research
- Work in partnership with:
- University of the West of England (UWE)
- BNSSG Integrated Care Board
- Local Authority Technology Teams
- Sirona Care and Health
- Assist in data collection and analysis to evaluate the effectiveness of the Personalised Medication Support Service.
3. Home Visits & Technology Implementation
- Conduct one-on-one home visits to evaluate individual medication routines.
- Offer guidance on selecting the best technology solutions suited to the service user.
- Oversee installation, troubleshooting and training for technology tools used in medication management.
- Monitor service users’ engagement with the technology and adjust solutions if necessary.
4. Ongoing Assessment & Project Evaluation
- Track service users’ progress over time through follow-up visits.
- Gather feedback on technology usability and effectiveness.
- Maintain detailed records to inform the broader evaluation of the pilot project.
5. Caseload Management & Prioritization
- Effectively organize and manage multiple service users, ensuring each receives appropriate support.
- Adapt to varied settings, including home environments, hospitals and care facilities.
6. Meeting Performance Targets
- Work towards specific performance benchmarks outlined by line managers.
- Support the achievement of key project objectives, ensuring timely and effective delivery.
